In-State Patients Only: The clinical services, medical consultations, and lab ordering offered by this Practice are strictly limited to individuals who are physically located within the State of Iowa at the time of service.
Out-of-State Restriction: We do not provide medical advice, establish doctor-patient relationships, or ship restricted functional lab kits to individuals residing or physically located outside of Iowa.
4. Intellectual Property

You may not reproduce, distribute, or use our content for commercial purposes without prior written permission. You may download one copy for personal, non-commercial use, provided you give full attribution.

6. Fees, Billing, and Individual Visit Bookings
Direct Payment Model: This Practice operates on a cash-based, direct-pay model. We do not participate in, accept, or bill any private health insurance plans, managed care networks, Medicare, or Medicaid.
Patient Responsibility: You are solely responsible for full payment of all services, consultations, lab fees, and supplements at the time they are booked or rendered.
Credit Card Transaction: There is a 3% processing fee for all non-cash or check transactions
Cancellation and No-Show Policy: You must provide at least 24 hours' notice to reschedule an individual visit. Missed appointments (no-shows), are non-refundable.
Insurance Reimbursement Disclaimer: We make no representations or guarantees that your insurance provider will reimburse you for any services or testing ordered by this Practice. We do not provide insurance claim codes (CPT/ICD-10) or look up coverage details for you.
Medicare/Medicaid Opt-Out: If you are a Medicare or Medicaid beneficiary, you acknowledge that our providers have opted out of these programs. You agree that you cannot submit claims to Medicare or Medicaid for any services received here.
7. Functional Lab Testing Purchases
Third-Party Lab Fees: The cost of specialized functional lab kits may be billed directly by the third-party laboratory or collected by our Practice. You are responsible for all processing or shipping fees associated with these tests.
Lab Kits Are Non-Refundable: Once a functional lab kit has been ordered, shipped to you, or picked up, the purchase is 100% non-refundable.
Expiration of Test Kits: Most functional lab kits must be completed and submitted to the laboratory within 3 months of purchase. The Practice is not responsible for expired, lost, or damaged test kits.
8. Supplement Sales and Return Policy
No Medical Guarantee: Recommendation of specific nutritional supplements does not constitute a medical prescription or guarantee of a specific health outcome.
Return Window: Unopened, unsealed, and undamaged supplement bottles may be returned within 14 days of purchase for a full refund, excluding shipping costs.
Final Sale Items: Opened supplements, refrigerated products (such as probiotics), and customized supplement protocols are strictly final sale and cannot be returned or exchanged.
Shipping Terms: For supplements purchased directly through our website, we ship exclusively to physical addresses within the State of Iowa. Risk of loss and title for items purchased pass to you upon our delivery to the shipping carrier.
Local Pickup Policy: If you choose local pickup for your supplements or lab kits, items must be collected from our office within 14 days of order confirmation. Unclaimed orders after this period may be non-refundable
9. Limitation of Liability & Indemnification
To the fullest extent permitted by law, the Practice shall not be liable for any direct, indirect, or consequential damages arising from your use of (or inability to use) our website or services. You agree to indemnify, defend, and hold harmless the Practice and its affiliates from any claims, liabilities, and expenses (including attorneys' fees) arising out of your misuse of the site or violation of these terms.

No Local Interruption Guarantee: While we strive to keep our website, patient portal, and supplement shop accessible, we do not guarantee uninterrupted access. We are not liable for any local internet outages, server downtimes, or technical glitches that prevent you from booking visits or purchasing items.
11. Governing Law and Jurisdiction
State Jurisdiction: These Terms and Conditions, and any disputes arising from your use of this website or our services, are governed exclusively by the laws of the State of Iowa, without regard to its conflict of law principles.
Local Venue: You agree that any legal action, suit, or proceeding arising out of or relating to these Terms must be filed solely and exclusively in the state or federal courts located in Johnson Country, Iowa. You hereby consent to the personal jurisdiction of these courts.
